Jacob Rogers, PharmD, BCACP, CDCES, is a board-certified ambulatory care clinical pharmacist and certified diabetes care and education specialist. A graduate of the Texas Tech Health Sciences Center School of Pharmacy with advanced residency training, he is passionate about helping patients manage diabetes, reduce cardiovascular risk, optimize medication therapy through deprescribing when appropriate, and overcome barriers to medication access and adherence.
