Melissa Baker, AGPCNP-BC, is a board-certified adult geriatric primary care nurse practitioner with more than 25 years of nursing experience prior to becoming an NP. Having completed a nurse practitioner preceptorship at the UNT Health Center for Older Adults, she provides compassionate, patient-centered care for older adults through the Health Pavilion, house calls and Fort Worth’s premier continuing care communities, with same-day and next-day appointment availability. Dedicated to truly listening to patients with complex needs, she focuses on what matters most to each individual in their care.
