Asset Resource Guide
South Asian Type 2 Diabetes – Texas Asset Resource Guide
Geographic scope: Texas (statewide), with concentration in the DFW Metroplex.
Frameworks applied: Oregon Community Engagement Behavioral Health Alliance (OCEBHA) categorized inventory model; UCLA Center for Health Policy Research, Section 1: Asset Mapping (Berkowitz & Wadud)
Purpose: This guide describes the assets in Texas that support South Asian Type 2 Diabetes (T2D) prevention, education, and management. Each asset has been included because it offers services, programming, or community infrastructure that meets the specialized needs of Indian, Pakistani, Bangladeshi, Sri Lankan, Nepali, and Bhutanese communities that experience T2D at roughly twice the rate of non-Hispanic Whites and at lower Body Mass Index (BMI) thresholds and earlier ages than other groups.
Texas has a layered network of health services relevant to South Asian T2D care, ranging from large academic medical centers with dedicated South Asian cardiometabolic programs (UT Southwestern, Baylor Scott & White) to community-based free clinics operating inside faith venues (TIPS Charity Clinic, ICNA Relief). Together they cover the full spectrum from prevention and screening to specialty endocrinology and complication management. Two organizations MHISA and SAMHIN extend the network into mental health, which is increasingly recognized as a critical co-factor in T2D self-management for South Asian populations. Statewide coordination is anchored by Texas Health and Human Services, which curates verified, multilingual diabetes education materials for residents and providers.

Provider & Program Finder Tools
A recurring goal for Community Health Workers (CHWs) in this protocol is connecting South Asian community members to the right healthcare professional: primary care physicians (PCPs), nurse practitioners, endocrinologists, registered dietitians, psychologists/therapists, podiatrists, cardiologists, nephrologists, and ophthalmologists, as well as to outpatient diabetes education programs available in their community or city. The tools below are national or regional locator services CHWs can use to make those referrals; several also allow filtering by language or cultural background.

They span culturally tailored faith-venue health fairs (BAPS Charities), South-Asian-specific clinical lifestyle programs (STOP-D, India House DPP), state-level coordination (Texas DSHS Diabetes Prevention and Control Program), regional initiatives in the Rio Grande Valley where T2D prevalence reaches ~30% (STDOI, Texas A&M, Salud y Vida, South Texas Diabetes Initiative), and the American Diabetes Association's North Texas chapter, which provides ready-made community programming and youth education infrastructure. Several of these programs already operate on a scale and have established protocols that can host or be adapted for Project-Saksham-style peer mentorship.

Social networks are the connective tissue of the asset map they reach South Asians who may not engage clinical or faith institutions, but who carry the same elevated T2D risk. The networks below cover four distinct pathways: (1) clinical-professional networks like IANANT that provide expert volunteer capacity for diabetes education; (2) senior- and women-focused networks (SAIVA, Daya, CHETNA) that serve high-T2D-risk populations facing isolation, caregiving stress, or domestic violence, all known social determinants of poor diabetes outcomes; (3) identity-affirming networks (KHUSHATX) that reach LGBTQ+ South Asians, a group facing compounded stigma in healthcare access; and (4) civic and pan-Asian advocacy networks (Asian Texans for Justice, Asian Chamber of Texas, NAAO) that provide statewide reach, business community linkages, and policy advocacy for community-level diabetes prevention infrastructure. Together they form a referral and amplification layer no clinical asset can replicate alone.

North Texas Tamil Sangam (NTTS) Mind Matters Series |
Tamil-language community wellness programming including the Mind Matters health education series; culturally tailored channel for reaching Tamil-speaking South Asians with diabetes prevention and wellness information. |
DFW, TX |
Secular cultural organizations are an indispensable layer of the asset map: they convene South Asians who do not regularly attend faith venues, including secular, mixed-faith, and second-generation members, and they organize the major non-religious gatherings (national-day celebrations, language-day events, festivals, sports leagues, youth competitions, senior socials) where T2D screenings and health education can reach hundreds of attendees in a single afternoon. Critically, these organizations are usually language- or country-of-origin-defined, which means partnering with them gives the CEAL protocol a near-perfect channel for linguistically and culturally tailored interventions: Telugu programming through TANTEX, Tamil through MTS and NTTS, Malayalam through Kerala Association of Dallas, Bengali through BANT, Urdu through PSNT and PCANT, Gujarati through DFW Gujarati Samaj, and pan-Indian umbrella outreach through IANT (a 1962-founded 501(c)(3) recognized as the FIA umbrella for DFW). Many already host annual health fairs, senior gatherings, and women's-wing programming, natural insertion points for diabetes screening tables and Project-Saksham-style peer mentorship. They are also the most cost-effective way to reach older first-generation immigrants (the highest T2D-risk subgroup) because cultural-org events draw multi-generational families and are typically free or low-cost.

Hindu Temples
The Hindu landscape in DFW is the largest and most internally diverse of the South Asian faith communities, with at least four temples drawing weekend congregations of several thousand. The DFW Hindu Temple Society (Ekta Mandir) in Irving is the centerpiece pan-Hindu, founded in 1990, and uniquely valuable to a T2D protocol because it already houses the TIPS Charity Clinic on its campus, giving CEAL a turnkey clinical partner inside a faith venue. The BAPS Shri Swaminarayan Mandir in Irving is the second anchor: a Gujarati Swaminarayan congregation that runs a structured annual BAPS Charities Health Fair with diabetes, blood pressure, BMI, and cholesterol screenings, a model that can be expanded into year-round Saksham-style mentorship. Karya Siddhi Hanuman Temple in Frisco is the largest Collin County Hindu venue and the natural outreach node for the Frisco/Plano professional South Asian population. Radha Krishna Temple of Dallas (Allen) serves the eastern Collin Vaishnava community. The Hindu Temple of Greater Fort Worth is the only major Hindu venue in Tarrant County and is therefore the indispensable Fort Worth partner. Two language-specific temples deserve priority for linguistically tailored content: Sri Guruvayurappan Temple in Carrollton (Kerala/Malayalee) and the Radha Kalachandji ISKCON Temple in Dallas, which additionally operates an on-site vegetarian restaurant, a natural site for nutrition counseling.

Mosques (Masajid)
The DFW Muslim South Asian population, predominantly Indo-Pakistani, with growing Bangladeshi and Ahmadiyya subcommunities, is anchored by three large mosques and one explicitly faith-grounded free clinic. East Plano Islamic Center (EPIC) is the dominant convening venue for the Plano corridor's Indo-Pakistani professional community and is the single most important T2D outreach mosque in Collin County by congregation size. Islamic Association of North Texas (IANT) in Richardson is the oldest large mosque in DFW (founded 1969), with established health and women's committees that have hosted Ramadan-tied health-education events; its institutional capacity makes it the natural partner for ongoing programming rather than one-off fairs. The Islamic Center of Irving (ICI) has served Irving's Indo-Pakistani and pan-Muslim population since 1991 and is the natural partner for the Irving cluster. Masjid Baitul Ikram in Allen (inaugurated 2022) serves the Ahmadiyya Muslim Community, which globally runs Humanity First medical initiatives, a built-in health-services partner. The ICNA Relief Medical Clinic in Dallas is the most operationally important Muslim health asset in the region: an active, Muslim-led free clinic delivering chronic disease management (including diabetes) to uninsured patients.

Sikh Gurdwaras
DFW's Punjabi Sikh community of roughly 12,000+ is concentrated in Irving, Richardson, and Garland and is served by three established gurdwaras, all with verifiable websites or phone contacts. The Gurdwara Singh Sabha of North Texas in Richardson (founded 1984) is the largest and oldest and operates a weekly langar (free community meal) a setting that lends itself naturally to nutrition education and culturally sensitive dietary counseling for T2D. Gurdwara Nishkam Seva in Irving publishes an active service schedule (Sunday 11:30am, Friday 7:30pm) and runs strong service-oriented programming, making it well-suited for volunteer-driven peer mentorship. The Sikh Temple of North Texas in Garland (founded 1976) is the longest-established East Dallas anchor and the natural partner for outreach to the Garland Sikh community.

Ismaili Centers (Shia Imami Ismaili Muslim)
The Ismaili community in North Texas is small but exceptionally well-organized: the global Aga Khan Health Board operates structured, evidence-based health programs through every Jamatkhana, making this one of the most receptive faith partners for a CEAL T2D protocol. The Ismaili Community Center & Jamatkhana in Plano is the local convening point and should be the entry partner. The Houston regional Jamatkhana sits outside the boundary but is included as a regional resource because the Aga Khan Council for the Southwestern U.S. coordinates programming between the two and can amplify a North Texas pilot.

Christian Churches with South Asian Congregations
Indian Christianity in North Texas is largely Malayalee (Kerala-origin), Mar Thoma, Orthodox, Knanaya, and Pentecostal, and these congregations are tightly knit, with services in Malayalam and English. They are historically under-mapped in South Asian health research even though their members carry the same elevated T2D risk. Sehion Mar Thoma Church in Plano is the largest Mar Thoma parish in DFW with an active diocesan health committee and a published phone line, a strong tier-2 partner. St. Thomas Orthodox Church (Malankara) in Dallas is listed and verifiable through its Diocese of South-West America parish directory. The three Indian Pentecostal Church (IPC) congregations, Tabernacle (Dallas), Shalom (Garland), and Hebron, together cover the East Dallas/Garland Malayalee Pentecostal community; IPC Shalom additionally runs bilingual services that reach second-generation members who may otherwise be missed.

Jain Temples
The DFW Jain community is small but exceptionally health-aware: a largely Gujarati congregation with predominantly vegetarian and vegan dietary practices, which makes nutrition-counseling alignment unusually strong. The Jain Society of North Texas (JSNT) Derasar in Dallas (founded 1982, 501(c)(3) #75-1924611) is the only major North Texas Jain venue with a verifiable website and address; it is non-sectarian (combining Shvetambara, Digambara, and Sthanakvasi practice) and serves the entire community. Siddhayatan Tirth in Windom, about 90 miles northeast of Dallas, falls outside the primary boundary but is included as an exception because it functions as the regional Jain pilgrimage retreat for North Texas residents and is a credible site for periodic wellness retreats.

Buddhist Temples (South Asian origin)
The South Asian Buddhist presence in North Texas is small and concentrated in the Sri Lankan Theravada community. The DFW Buddhist Vihara in Fort Worth is the only verifiable, contactable South Asian-origin Buddhist temple in the region, with a published phone, email, and active website. It is therefore the single point of partnership for outreach to the Sri Lankan diaspora, a community that, like other South Asian groups, carries elevated T2D risk but is rarely included in U.S. South Asian health research because of its small absolute numbers.

Cross-Cutting Health Programming Already Operating in Faith Spaces
Three programs are already bridging faith venues and T2D care in DFW and should be primary CEAL partner candidates:
- TIPS Charity Clinic - operating inside the DFW Hindu Temple (Irving). Free clinic with chronic disease management.
- BAPS Charities Health Fair - annual at the BAPS Mandir (Irving), with diabetes, BP, cholesterol, and BMI screenings (org).
- ICNA Relief Medical Clinic Dallas - Muslim-led free clinic with active diabetes management (org/dallas-clinic).
